OSHA Fines Subcontractor After Iatan Plant Death

KANSAS CITY, Mo. - A company faces more than $14,000 in fines after a fatal accident earlier this year at a power plant construction site.

The Occupational Safety and Health Administration notified Team Industrial Services, Inc. about the fines last week.

Investigators found two alleged serious violations after the March 31 accident at the Iatan 2 construction site in Weston, Mo.  The accident happened about 5:45 a.m. inside the boiler at the site.  

28-year-old Vatthana Vongtheva of Emporia was using an X-ray device to make sure welds were done properly on the boiler. No details have been released about the nature of the accident that killed Vongtheva.

The citations allege employees were exposed to hazards entering and exiting a suspended scaffold and were required to enter the scaffold platform by crawling over the scaffold railing point of access.  The second citation alleges employees were exposed to hazards using a suspended scaffold that was not inspected daily to assure the scaffold did not have any visible defects.

The power plant construction site is next to Iatan 1, the plant that helps supply power to KCPL customers.

The company will have the opportunity to appeal the fines.
